Mini Facelift Surgery

The mini facelift is one of facial plastic surgery’s most patient-selective procedures — it produces excellent results for the right candidate and disappointing ones for the wrong candidate. Also known as the short scar facelift, the procedure uses smaller incisions and a less extensive surgical dissection than a traditional facelift, primarily addressing the midface area and the lower two-thirds of the face. It is well-suited to patients in their late 30s, 40s, and early 50s who have begun to notice early signs of aging — particularly creeping wrinkles, the development of midface descent, and softening around the nasolabial folds — but who are not yet ready for a comprehensive facelift. The mini facelift is one of the surgical options within facelifts and facial rejuvenation at Dr. Khosh’s Park Avenue practice in New York City.

The mini facelift succeeds when the surgeon’s judgment about candidacy is correct — and disappoints when it isn’t. Dr. Maurice Khosh has spent three decades developing exactly that selection judgment across the full spectrum of facelift procedures. Dual board-certified by the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology–Head and Neck Surgery and a Fellow of the American College of Surgeons (FACS), he has been recognized as a perennial Castle Connolly Top Doctor, a Best Doctors in America honoree, and a Patients’ Choice Award recipient.

About the Short Scar Facelift

The mini facelift takes its name from the limited scope of its incisions and the gentler nature of its surgical dissection compared to a traditional facelift. The procedure uses smaller incisions — usually around the ears only, without extending into the hairline as a full facelift does — which is why it is often referred to as the short scar facelift. The technique addresses the midface area, frown lines around the nose and mouth, and mild jowl development, lifting and repositioning the lower two-thirds of the face for a refreshed, younger appearance.

Today’s approach to facelifts is increasingly focused on maintaining a youthful appearance rather than waiting until major surgery becomes necessary to reduce significant signs of aging. The mini facelift is a perfect example of using surgical rejuvenation to address the inevitable signs of aging before more complex facelift surgery becomes the only option.

Mini Facelift vs. Traditional Facelift

The most important practical distinction between the mini facelift and a traditional facelift is the scope of what each can address. A traditional facelift — particularly a deep plane facelift — engages the deep tissue layers and the supporting ligaments of the face, addressing comprehensive aging changes throughout the face and often the neck. A mini facelift uses smaller incisions, less extensive dissection, and a more targeted scope of correction. Because the mini facelift does not address excess sagging under the chin or on the neck, it is not recommended for patients with significant neck laxity, prominent platysmal banding, or substantial double-chin concerns. For those patients, a neck lift or full facelift remains the more appropriate option.

Who Is a Good Candidate

The mini facelift is best suited to patients with one or more of the following characteristics:

  • Early to Moderate Signs of Aging: Wrinkles around the midface, beginning of jowl development, or early softening of the jawline
  • Late 30s to Early 50s: Patients whose aging changes are present but not yet advanced
  • Good Skin Elasticity: Skin that will redrape smoothly after the lift, producing a natural result
  • Minimal Neck Laxity: Patients whose primary concerns are in the lower two-thirds of the face, not the neck
  • Realistic Expectations: Patients who understand that the mini facelift slows the signs of aging rather than turning them back
  • A Desire for Less Invasive Surgery: Those who want meaningful improvement without the recovery of a traditional facelift

Patients with substantial midface descent, significant skin laxity, prominent platysmal banding, or advanced aging changes are typically better served by a deep plane facelift, midface lift, or neck lift — sometimes in combination. Dr. Khosh evaluates each patient honestly to recommend the procedure that will actually deliver the result they are seeking.

“The mini facelift is one of those procedures where the most important conversation is honest patient selection. For someone with early aging changes and good skin elasticity, it can deliver a meaningful, natural refresh with significantly less recovery. For someone with advanced changes, it delivers an underwhelming result that doesn’t last. Matching the procedure to the actual anatomy is the entire point.” — Dr. Maurice Khosh

Benefits of the Mini Facelift

Patients who are good candidates for a mini facelift typically experience several advantages compared to a traditional facelift:

  • Smaller, Shorter Incisions: The short scar technique uses incisions limited to the area around the ears, producing less visible scarring
  • Faster Recovery: Most patients return to social activities within seven to fourteen days, rather than the two to three weeks typical of a full facelift
  • Less Anesthesia: Often performed under local anesthesia with sedation rather than general anesthesia
  • Lower Cost: A more accessible price point than a full facelift, while still delivering meaningful improvement
  • Preventive Approach: Addresses early aging changes before they progress to the point where more extensive surgery becomes necessary
  • Natural-Looking Results: When performed on the right candidate, produces a subtle refresh without the appearance of having had work done

Recovery and Results

Mini facelift recovery is significantly faster than traditional facelift recovery. Most patients experience mild to moderate swelling and bruising for the first one to two weeks, with bruising typically resolving within seven to ten days. Most patients return to work and social activities within one to two weeks, with strenuous activity avoided for three weeks. Results typically last five to ten years depending on age at the time of surgery, skin quality, and lifestyle factors. While the mini facelift does not last as long as a deep plane facelift, it can be an ideal first step that delays the need for more comprehensive surgery later.

Saw Dr. Khosh for a fractured nose. He reset my nose beautifully (and with little discomfort to me) and then discussed some of my sinus issues with me upon my follow up visit. Never felt like he was rushing me out and felt very confident in his abilities. I would highly recommend to anyone needing plastic or reconstructive options!

Upon hearing of the overwhelmingly positive experience a friend of mine had in choosing Dr. Khosh as her surgeon, I decided to make an appointment for my own septoplasty/rhinoplasty/turbinate reduction. Although I was slightly skeptical as the praise I had heard seemed a bit hyperbolic, all uncertainty dissipated after meeting the Doctor and his staff. My pre- and post-operative experience was excellent in terms of concerns adequately addressed prior to the surgery and mitigation of any bruising in the convalescent period. Despite my apparent predisposition to rather slower healing, a year after the surgery, my nose looks really pretty and my breathing entirely unobstructed! I highly recommend Dr. Khosh!

AMAZING!!!!!! I had Dr Khosh perform lipo on my chin/neck and my result is unbelievable! Better than I could have expected! I met with a couple of surgeons and the second I met with Dr Khosh I knew he was the one I was going with. He was compassionate and explained every detail of what would take place during the surgery and what to expect during recovery. I even emailed a few times during recovery with various questions and he got right back to me asap which was really comforting! I honestly fully recommend Dr Khosh, he did an amazing job and is an amazing person.
