Lip Dermal Fat Grafting
Lip dermal fat grafting is one of the most natural and longest-lasting ways to enhance the fullness of your lips — and the only major lip augmentation option that uses the patient’s own tissue rather than synthetic filler. The procedure harvests fat from a donor area on the body (typically the abdomen or hips) and transfers it into the lip tissue, where the grafted fat establishes a new blood supply and integrates as living tissue. Unlike hyaluronic acid fillers, which gradually dissolve over six to twelve months, properly grafted fat can last for years — sometimes permanently, as the lips become subtly fuller and more youthful with the patient’s own restored volume. As fat naturally diminishes with age in the lips just as it does elsewhere on the body, dermal fat grafting offers a way to restore the lost volume using the patient’s own biological material.
Two distinct technical challenges define every successful lip fat grafting procedure: how the fat is harvested from the donor site, and how it’s placed in the lip tissue to produce a smooth, lasting result. Dr. Maurice Khosh’s three decades of facial plastic surgery have refined both. Dual board-certified by the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology–Head and Neck Surgery, a past president of the New York Facial Plastic Surgery Society, and a Fellow of the American College of Surgeons (FACS), Dr. Khosh has been recognized as a perennial Castle Connolly Top Doctor.
Why Fat Grafting Lasts Longer Than Fillers
The fundamental difference between fat grafting and synthetic filler injection is that fat is living tissue. When properly harvested and placed, the grafted fat cells establish their own blood supply in their new location and integrate with the surrounding lip tissue. Once that integration is complete — typically within the first three to six months — the grafted fat behaves like any other fat on the body: it stays in place, responds to the patient’s overall body composition, and provides volume that does not gradually dissolve the way hyaluronic acid filler does. Most patients experience some natural absorption in the first weeks after the procedure (typically 30 to 50 percent of the grafted volume), with the remaining fat establishing as long-term volume that can last for years.
The Two-Step Procedure: Harvest and Placement
Successful fat grafting depends on careful execution of two distinct surgical steps:
- Harvest from the Donor Site: Fat is gently extracted from the abdomen or hips using specialized small-diameter cannulas designed to preserve the integrity of the individual fat cells; the gentleness of the harvest directly affects how much of the grafted fat survives the transfer
- Processing of the Harvested Fat: The extracted fat is carefully processed to separate out fluid, blood, and damaged cells, leaving concentrated viable fat ready for transfer
- Precise Placement in the Lip: The processed fat is placed in small aliquots throughout the lip tissue using fine cannulas, with attention to symmetry, lip border definition, and overall lip shape
- Symmetry Verification: Dr. Khosh checks symmetry throughout the procedure to ensure a balanced result
“The success rate of fat grafting depends almost entirely on how gently the fat is treated during harvest and how carefully it’s placed afterward. Fat cells are delicate — handling them too roughly during the harvest, or placing them in clumps that exceed what the surrounding tissue can vascularize, will lead to absorption. The technique determines the durability.” — Dr. Maurice Khosh
What to Expect from the Natural Lip Plumping Procedure
Lip dermal fat grafting is typically performed under local anesthesia with light sedation, or under general anesthesia for patients who prefer it. The procedure usually takes one to two hours depending on the donor site selected and whether the patient is having other procedures performed simultaneously. Patients return home the same day. Most experience moderate swelling and minor bruising in both the donor area and the lips for one to two weeks. The lips will appear over-augmented initially due to swelling; the final result becomes visible as the swelling resolves and the grafted fat settles, typically at three to six months. Some patients require a touch-up procedure to optimize the final result.
