A defined, balanced jawline is one of the most influential features of overall facial harmony. When the masseter muscle — the powerful jaw muscle responsible for chewing — is enlarged from genetics, chronic teeth grinding, or repetitive clenching, it can create a wider, squarer lower face that distracts from the rest of the features. Botox for masseter reduction is a non-surgical, in-office treatment that gradually softens the appearance of an overdeveloped jaw, producing a slimmer, more tapered profile without surgery, incisions, or downtime.

Dr. Maurice Khosh is among the few facial plastic surgeons in New York City whose training spans both the aesthetic and the functional anatomy of the jaw. Dual board-certified by the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology–Head and Neck Surgery, he is a Fellow of the American College of Surgeons (FACS) and an active member of the American Academy of Facial Plastic and Reconstructive Surgery, the American Academy of Otolaryngology–Head and Neck Surgery, and the American College of Surgeons. Recognized as a Castle Connolly Top Doctor, a Best Doctors in America honoree, and named a Super Doctors New York Rising Star, Dr. Khosh combines surgical-level anatomical knowledge with the artistic eye of a lifelong figure-drawing and calligraphy student — a background that informs the precision and restraint he brings to facial proportion work.

The Anatomy Behind Masseter Botox

The masseter is one of the strongest muscles in the human body, and one of the largest muscles in the face. It originates on the cheekbone and inserts along the angle of the jaw, where its size and bulk directly shape the contour of the lower face. Reducing it effectively requires more than knowing where to inject — it requires understanding the muscle’s relationship to surrounding structures, including the parotid gland, the facial nerve branches, and the buccal fat pad. Improper placement can cause asymmetry, an uneven smile, or, in rare cases, a “smile droop” if Botox migrates into neighboring muscles.

“The masseter is one of the strongest muscles in the body — its size, position, and how it interacts with the surrounding facial structure all influence where and how much Botox should be placed. Treating it well requires a working knowledge of head and neck anatomy, not just an aesthetic eye.” — Dr. Maurice Khosh

What Botox for Masseter Reduction Addresses

Masseter Botox is best suited to patients who feel their lower face is wider than their upper face, those with a strong square or “boxy” jawline, and those whose facial proportions feel out of balance because of jaw bulk. It is also frequently chosen by patients who experience the functional consequences of an overactive masseter — including:

  • Wide or Square Jawline: Genetically enlarged masseter muscles that create a heavier lower-face appearance
  • Bruxism and Teeth Grinding: Chronic clenching that has thickened the muscle and contributes to tooth wear
  • TMJ-Related Tension: Jaw soreness, tightness, or tension headaches associated with masseter overactivity
  • Facial Asymmetry: Uneven masseter development that makes one side of the jaw appear fuller than the other
  • Loss of Lower-Face Definition: Bulkier jaw contours that obscure cheekbone definition and overall facial taper

What to Expect During Treatment

Masseter Botox is performed in Dr. Khosh’s Park Avenue office and typically takes less than ten minutes. After cleansing the skin and marking the injection points, Dr. Khosh injects a calibrated dose of Botox — usually 1 to 3 injections per side — directly into the belly of the masseter muscle. Most patients describe only a brief pinch with each injection, and there is no recovery period required. Initial muscle softening begins within 1 to 2 weeks, with the slimming effect becoming visible at 4 to 6 weeks and continuing to refine for up to three months. Results generally last 4 to 6 months, with many patients finding that the muscle responds more efficiently to repeat treatments and requires less frequent dosing over time.

Frequently Asked Questions About Botox for Masseter Reduction

How long do the results from masseter Botox last?

Most patients enjoy noticeable jaw slimming for 4 to 6 months after treatment. Because the masseter is a large, frequently used muscle, results can vary based on muscle strength and individual metabolism. With consistent treatments over time, many patients find their masseter responds more effectively and the slimming effect becomes longer-lasting.

When will I see results?

Botox typically takes 3 to 7 days to begin softening the muscle, with visible slimming developing gradually over 4 to 6 weeks. Because the change happens slowly through muscle atrophy rather than immediately, results look natural and avoid the sudden change often associated with other facial treatments.

Is the treatment painful?

Most patients tolerate masseter Botox very well. Dr. Khosh uses a fine-gauge needle and a calibrated injection technique, and topical numbing is available on request. Most patients describe the sensation as a brief pinch, and there is no downtime — you can return to work and normal activities immediately.

Can masseter Botox help with teeth grinding or TMJ symptoms?

Yes — many patients seek masseter Botox specifically for the functional benefits. By reducing the strength of the masseter contraction, Botox can ease bruxism (teeth grinding), reduce jaw tension, and relieve associated symptoms such as tension headaches and morning jaw soreness. This is one reason Dr. Khosh’s head and neck surgery training is especially relevant for evaluating candidates.

Will my ability to chew or speak be affected?

No. The masseter is one of several muscles involved in chewing, and Botox reduces — rather than eliminates — its strength. Normal eating, speaking, and facial expression remain comfortable. Some patients notice mild fatigue when chewing very tough foods for a few days after their first treatment; this typically resolves quickly as the surrounding muscles adapt.

How does masseter Botox compare to surgical jaw reduction?

Surgical jaw reduction is a more invasive option that involves shaving or reshaping the bone of the mandible itself, with significant recovery and risk. Masseter Botox slims the lower face by reducing muscle bulk rather than altering bone, offering a gradual, reversible, no-downtime alternative for patients whose jaw width comes primarily from muscle rather than bone structure. Dr. Khosh evaluates the underlying source of jaw bulk during consultation to determine which approach is most appropriate.

Why Patients Choose Dr. Khosh for Masseter Botox

  • Surgeon-Level Anatomy: Treatment planned by a dual board-certified facial plastic surgeon with deep head and neck training
  • Precise Injection Placement: Calibrated dosing avoids common complications such as smile asymmetry or uneven softening
  • Aesthetic and Functional Balance: Treats both the cosmetic profile and the underlying bruxism or TMJ contribution
  • Artistic Sensibility: A facial proportion philosophy guided by years of figure drawing and an eye for natural balance
  • Park Avenue Location: Discreet, private practice on the Upper East Side serving patients from across the tri-state area

Saw Dr. Khosh for a fractured nose. He reset my nose beautifully (and with little discomfort to me) and then discussed some of my sinus issues with me upon my follow up visit. Never felt like he was rushing me out and felt very confident in his abilities. I would highly recommend to anyone needing plastic or reconstructive options!

Upon hearing of the overwhelmingly positive experience a friend of mine had in choosing Dr. Khosh as her surgeon, I decided to make an appointment for my own septoplasty/rhinoplasty/turbinate reduction. Although I was slightly skeptical as the praise I had heard seemed a bit hyperbolic, all uncertainty dissipated after meeting the Doctor and his staff. My pre- and post-operative experience was excellent in terms of concerns adequately addressed prior to the surgery and mitigation of any bruising in the convalescent period. Despite my apparent predisposition to rather slower healing, a year after the surgery, my nose looks really pretty and my breathing entirely unobstructed! I highly recommend Dr. Khosh!

AMAZING!!!!!! I had Dr Khosh perform lipo on my chin/neck and my result is unbelievable! Better than I could have expected! I met with a couple of surgeons and the second I met with Dr Khosh I knew he was the one I was going with. He was compassionate and explained every detail of what would take place during the surgery and what to expect during recovery. I even emailed a few times during recovery with various questions and he got right back to me asap which was really comforting! I honestly fully recommend Dr Khosh, he did an amazing job and is an amazing person.
